Attention Deficit Disorder Treatment Instructions

Thank you for entrusting us with your child’s Attention Deficit Disorder management. We strive to provide consistent and reliable care for your child. To optimize your child’s treatment, please help us by adhering to the following guidelines:

1. Regular rechecks are required to ensure that your child is doing well on the medication with minimal side effects. We require that your child is checked in the office within 4-6 weeks of starting the medication, and then a minimum of every 6 months after that (the yearly well checks will count as one of these visits). We will advise you if the child needs more frequent follow-up. We ask that you have Vanderbilt forms filled out and returned to us before each recheck.

2. We ask that you make a request for medication refills a minimum of 5 days prior to running out of medication. This allows us the time it takes to review each chart, arrange for further follow-up if necessary, and to write the requested prescription as appropriate. If you call fewer than 5 days prior to running out of medication, your request will be handled as quickly as possible.

3. Because these medications are controlled substances, we request that you bring a photo ID when you pick up your child’s medication. Children under18 years of age are not allowed to pick up their own medications.
